Request Your Health Care Records
Under Texas law (HB 4224) and the Health and Safety Code Chapter 181, you have the right to request copies of your health care records from PrimeCare Emergency Center. We are committed to processing your request quickly and confidentially.
Step 1
Complete the "Authorization for Release of Health Information" form from the section above.
Step 2
Submit the completed form or send to our Medical Records department.
- In Person: Visit our front desk at 5912 S Cooper St, Suite 110, Arlington, TX.
Step 3: Processing and Delivery
- Turnaround Time: Requests are typically processed within 10-15 business days of receipt, as permitted by Texas law.
- Fees: A reasonable fee may be charged for reproduction of records (e.g., $25 for first 20 pages + $0.15 per page thereafter) and actual shipping costs for mailed copies.
- Delivery: You may pick up records in person, receive them via secure email, or have them mailed to your address on file.

As a consumer, you have the right to report false, misleading, or deceptive business practices to the Texas Office of the Attorney General (OAG). If you wish to file a complaint regarding billing, privacy, or services, please use the resources below.
