What to Bring for an Unscheduled Holiday Visit


Do not delay urgent treatment while searching for paperwork. If another person can gather your information safely, a few items can make registration and clinical review easier.

  • A driver’s license, state ID or other government-issued photo identification
  • Your current health insurance card, if you have one
  • A list of prescription medicines, over-the-counter products and supplements, including doses and how often you take them
  • Known medicine, food or contrast-material allergies
  • A short medical history covering major diagnoses, recent procedures and previous surgeries
  • Contact details for your primary doctor or relevant specialist
  • Advance directives or medical power-of-attorney documents when applicable
  • A phone charger and basic comfort items if observation may be needed

For a child, bring the parent or guardian’s ID, the child’s insurance information and an accurate medication list. If someone else has legal authority to consent to treatment, bring the relevant paperwork when it is readily available.
